Introduction
Fundació Privada per a la Recerca Sant Joan de Déu invites you to participate in "Science4Pandemics" project dedicated to research on the prevention and management of pandemics.
Benefits
Your participation in the project, playing the simulation game or answering opinion questionnaires will help to expand the knowledge we have about pandemics.
Contributing your experiences and opinions is part of citizen science. Citizen science is a collaboration between scientists and non-scientists, where citizens can contribute their own experiences and opinions to a specific topic that is being researched.
Your answers to our questions, and your possible interaction with the game (how you play and solve the challenges), will be used by the group of researchers and paediatricians at Hospital Sant Joan de Déu to complement the research studies that we are carrying out.
Voluntary Nature and Termination of the Study Participation
Your participation is voluntary. You can decide not to withdraw your participate anymore (not playing anymore or not answering other questionnaires) at any time without giving any explanations or reasoning.
Consent
By approving the following clauses, you give your consent to participate in the Science4Pandemiccs project and for us to:
• Analyze your answers to questionnaires
• Analyze your interactions in the game
• Store your responses in a secure database in an anonymized and supervised manner governed by data protection laws so that they can be used in future research.
• Allow the analysis of your answers, and interactions in the game by the research team
• Be contacted to share with me the results of the studies, as well as update me on the progress of the project and related initiatives.
After having read all information about the purposes and ultimate goals of SCIENCE4PANDEMICS, and after being given the opportunity to ask any questions via email (info@Science4Pandemics.eu), I confirm
I have been fully informed of the implications of my participation in the Science4Pandemics project; I fully understand the content of this document and I accept the offer to voluntarily participate in the Science4Pandemics project.

Purpose
Collect information on knowledge and preferences of society in the field of prevention and management of pandemics.
The Foundation will keep the data for the duration of the relationship with the interested party and the interested party does not request the right of cancellation. Inform that the Foundation will dispose of them in accordance with the established legal obligations, and once completed, will proceed to their elimination or anonymization.
Legal basis
The legal basis for data processing lies in the consent (9.2.a of the RGPD) of the interested party.
Addressee
No data is transferred to third parties, except in the case of legal obligation. With the exception of the inclusion of the data in the Science4Pandemics platform.
Rights
Access, rectification and cancellation of data, as well as other rights, as detailed in the Organic Law on Protection of Personal Data and Guarantee of Digital Rights (LOPDGDD)* and in Regulation (EU) 2016/679 of Parliament European and Council, of April 27, 2016, regarding the protection of natural persons with regard to the processing of personal data and the free circulation of these data (GDPR), must be exercised by written communication to the Fundació Privada per to Sant Joan de Déu Research (Santa Rosa 39-56 - 08950 Esplugues de Llobregat) or via email to dpd@sjdhospitalbarcelona.org (Mr. Eduard Puig).
If you consider that your rights have not been taken into account, you can file a claim directly with the Spanish Agency for Data Protection.
